Causes

The cause of cerebral AVM is unknown. An AVM occurs when arteries in the brain connect directly to nearby veins without having the normal small vessels (capillaries) between them. AVMs vary in size and location in the brain. An AVM rupture occurs because of pressure and damage to the blood vessel.

The treatment of brain arteriovenous malformations (AVMs) is challenging based on a multidisciplinary approach involving neurosurgery, radiosurgery, and interventional neuroradiology. The embolization of brain AVMS was for a long time performed using particles and glue. Onyx has been recently introduced in the armamentarium for AVM embolization, but large, multicenter evaluation is lacking. BRAVO study was conducted to evaluate the clinical and anatomical results of brain AVM embolization with Onyx.

What is brain AVM?

Brain Arteriovenous malformation is an abnormal connection between arteries and veins in the brain that provide low resistance passage to blood. This redirects oxygen supply from other parts of the brain and body to these paths where oxygen exchange does not take place.

What are the implications of brain AVM?

AV malformation causes decreased supply of oxygen to the brain and rest of the body. It can also lead to blood pressure changes and rupture to form brain haemorrhages due to the same.

What is brain AVM embolization with Onyx?

Brain AVM embolization is a procedure with which the connection between arteries and veins in the brain can be blocked or narrowed by using onyx. Hence, the mixing of arterial and venous blood is disrupted or reduced.

What is Onyx?

Onyx is a chemically non-reactive form of silica that is used in liquid form to disrupt the flow in the arteriovenous channels. It is safe and efficient in management of AV malformation. Other materials used are polyvinyl alcohol foam particles.

What are the surgeries done after AVM embolization with Onyx?

If AVM is only partially decreased, then surgery or radiosurgery (using a tool called gamma knife) is used to completely block the AV malformation.

What is the success rate of brain AVM embolization with Onyx?

In most studies, depending upon the size of the malformations, reduction in arteriovenous malformations ranged between 40-100%, 75% on an average.
