Bariatric surgical procedures cause weight loss by restricting the amount of food the stomach can hold, causing malabsorption of nutrients, or by a combination of both gastric restriction and malabsorption. Bariatric procedures also often cause hormonal changes. Most weight loss surgeries today are performed using minimally invasive techniques (laparoscopic surgery).

 

 

The most common bariatric surgery procedures are gastric bypass, sleeve gastrectomy, adjustable gastric band, and biliopancreatic diversion with duodenal switch. Each surgery has its own advantages and disadvantages.

What is gastric band and what are it's complications?

Gastric band is an application of a restrictive band on the stomach which will divide it into upper, smaller and lower large compartments so entry of food can be controlled and when one eats food he/she gets a feeling of early satiety and hence weight is controlled eventually.

What is gastric bypass and what are it's complications?

Gastric bypass is restrictive as well as malabsorptive method.Stomach is dissected or cut into two parts.Lower large portion is stapled. Also the intestine connected to the lower portion of stomach is cut and stapled and the other free end of intestine is connected with the upper smaller part of stomach either with suture or other method. So that food entry into the stomach can be controlled.

What is Sleeve Gastrectomy and what are it's complications?

The stomach is dissected in verticle direction into two portions and it is stappled to both portions.As the portion connected to the remaining part of the intestine is untouched hence no additional suturing is required but the surface area of stomach is reduced to half or less than that.
Complications:Leakage,Intra Abdominal bleed,Pulmonary embolism,Deep vein thrombosis,Gastroesophageal reflux,Weight regain

What is Biliopancreatic diversion with duodenal switch?

It is dissection of stomach where stomach is converted into small pouch and connected directly to distal intestine.Duodenum and jejunum are bypassed so that absorption of calorie and micronutrient is hampered.But to ensure metabolism of fats by bile and pancreatic juices duodenum and Jejunum are sutured to distal ileum so that transport of digestive juices is possible.

What are other treatments available?

Two or more procedures can be combined to give better results such as:
-Duodenal bypass with sleeve gastrectomy
-Gastrectomy and ileal transposition

What all investigations should be done before surgery?

Investigations like total blood count,LFT,RFT,Electrolyte and urea,Ferritin,Folate,Vitamin D,Serum calcium, Parathyroid hormone level,HBA1c,Fasting glucose and lipid profile are done for surgical fitness before surgery.
